4.Do you rent or own where you live? If rent is the landlord ok with you having pets?

5.How long have you lived at your current location?

6.Do you have a current vet? If yes can we have the name and number of the vet and do we have your permission to contact them?

7.Do you agree to keep your companion INDOORS only forever?

8.Do you agree to spay/neuter your kitten within 90 days of adoption?

9.Do you plan on declawing?

10.Do you plan on micro-chipping?

11.Do you agree to return the feline to the Midwest Himalayan Persian Connection if you are unable to keep it?

12.Do you agree to take care of your kitten by giving it proper shelter, food, fresh water, litter box and lots of love and affection?

13.Do you agree to keep up with the necessary daily grooming and face cleaning that is generally needed for at least the first year?
